This is counselling and psychotherapy with Millicent, who currently offers online video calls via Google Meet and in person session from Sunrise Wellbeing in Groby.

How your Personal Information is stored:

Initial Assessment Form, Contract/Agreement, Brief Session Notes, or Client Code (linking documents) are all stored in a locked filing cabinet.
Electronic:

Email Address, Email Correspondences, or you Social Media Account if this is how you chose to make contact. Your telephone number may be stored in Millicent's mobile (on a work sim under you initials), should messages be exchanged this way.
- Referral/Enquiry Form/Emails, Initial assessment Form, Updated Intake Form for contact details, Contract/Agreement, Session Notes, Client Code (linking documents), Invoices and an Evaluation/Ending Report.

Your contact details and date of enquiry on the contact page, will be emailed directly to Millicent password protected Gmail account, data is then is stored via a password-protected USB, stored in a locked filing cabinet.

Session payments are tracked through online banking.
How I may Process/Share/Keep your Personal Information:

The content of your session may be discussed in supervision in line with the British Association for Counselling & Psychotherapy (BACP) guidelines of ethical practice, your name and any identifying details will remain anonymous. This is in order to assist you further and ensure the support you receive is beneficial and accurate.

Millicent treats all personal data as confidential and does not pass this information onto third parties unless you express consent to disclosure, in an emergency to protect you or others, to comply with laws or to the courts.

Therapy with Millicent holds your details and clinical records for the duration of the therapeutic support and for 5 years post completion. if you wish to access your notes please let Millicent know. you will be required to verify your ID and provided your client code found on your contract form.

To ensure she provides the best possible therapeutic service, Millicent must hold your data to comply with legal obligations, including, establishment and defence of legal rights, and the prevention of crime.
